สำเนาบัตรประจำตัวประชาชนของบิดา-มารดา และผู้ปกครอง จำนวน 1 ชุด
สำเนาสูติบัตร จำนวน 1 ชุด
รูปถ่ายหน้าตรงขนาด 1 นิ้ว จำนวน 2 รูป
ใบรับรองผลการศึกษา (ปพ.7)
ระเบียนแสดงผลการเรียน (ปพ.1)
แบบแสดงการพัฒนาคุณลักษณะอันพึงประสงค์ (ปพ.4)

A copy of the student’s house registration – father – mother and evidence of name-surname change (in case of change) amount 1 set.
A copy of the identity card of the father-mother and parents, 1 set.
1 copy of birth certificate.
